Description

In 2016, NHS Fife and NHS Forth Valley began to pilot a new GP Community Hub Fellowship model. This aimed to bridge the gap between primary and secondary care for frail elderly patients and those with complex needs. Here you can download the evaluability assessment and evaluation.
